The authors present their experience and results of a combined treatment of aortic dissection with a brief review of literature. Eight patients were treated by endovascular aneurysm repair (EVAR) because of acute type B aortic dissection (DeBakey type III) between January 1997 and July 2001. Indications for endovascular treatment were: old age of patients, poor general condition and the old age of the aortic pathology. Direct aortic stent grafting (15-24 mm) with an endoprothesis was performed through femoral arteries. The type of endoprosthesis used, use of concomitant procedures and complications encountered are reviewed. Aortic dissection was confirmed by CT scan. No other cause of ascending aortic aneurysm was found. Size of the ascending aorta was 45 mm in one case, 44 mm in five cases and 45-50 mm in two cases. Endovascular treatment was technically successful in 7/8 patients. Stroke occurred in one patient; this patient also had neurological deficits. Hypertension persisted in 1/8, but was well controlled in 6/7 patients. Recurrent dissection after endovascular treatment was present in 3/8 patients. All these patients were medically treated. One patient died of infection 4 months after endovascular treatment. No periprocedural mortality was observed. All patients received an open surgical approach and remained in the hospital for 15-28 days. Postoperative complications occurred in 5/8 patients: 1/8 developed postoperative stroke, 3/8 thromboembolism (both legs), 1/8 cardiac failure and 1/8 was treated for wound infection.
